Its three packages cost €2,390, €3,290 and €5,790, and the difference between them is mainly who does the work: in the €2,390 Gold package, surgical technicians open the channels and place the grafts after a doctor plans the case; in Platinum, doctors open the channels; only in Diamond do the founding doctors or the senior head doctor perform the operation. Who performs the surgery at Smile Hair Clinic?

It depends on the package you buy, and Smile is unusually open about it. Its own price page explains that "the fundamental difference" between tiers "lies in the extent to which the procedure is performed personally by the doctor". In Gold, a doctor plans the case and designs the hairline, while surgical technicians make the sapphire incisions and implant the grafts. Platinum moves the incisions to the two named doctors. Diamond is performed by the founding doctors or the senior head doctor.

That openness is worth credit, and it also tells you what to check. The International Society of Hair Restoration Surgery (ISHRS) treats donor harvesting and recipient site creation as surgical steps for a licensed physician, and advises patients to ask whether anyone unlicensed will make incisions or harvest grafts. Which techniques does Smile Hair Clinic offer?

Sapphire FUE is the main technique, alongside DHI and unshaven or long-hair FUE for patients who do not want to shave. Smile's price page says unshaven and long-hair sessions are a medical decision rather than an upgrade on a price list. The ratings are very high and the volume is large. On Trustpilot, Smile has a TrustScore of 4.9 from 4,176 reviews, with 97% five-star and about 1% one-star ratings, on a profile claimed in October 2018; Trustpilot notes that the company has replied to 57% of negative reviews and has not invited customers recently. On Google Maps it held 4.9 at our last check. The clinic publishes a before-and-after gallery on its website, labelled by graft count and technique.

Use both carefully. A high average is common in this market, so read the one- and two-star reviews to see what goes wrong and how the clinic responds. In photos, look for results at 12 months, the same lighting and angle before and after, and cases similar to yours in hair type and Norwood stage.
